FUNCTION:
Under general direction of the Clinic Site Supervisor provides information and referrals for preconception health, pregnant and post-partum women who are within WIC Clinic Community or WIC eligible.
Work involves responsibility for case management including assessment, planning, advocacy and follow-up; coordinating, managing and evaluating assigned program services; networking with community agencies and developing outreach contact on behalf of and in support of clients. Work also involves participating in departmental task forces, staff meetings, community groups, meetings with business representatives and/or advisory councils. Supervision is received from an administrative superior with work being reviewed in the form of reports, conferences and effectiveness of program services provided.
TASKS AND RESPONSIBILITIES:
• Arrange client-confirmed services
• Monitor and document client needs, progress and success.
• Serve as family advocate when making referrals to community programs
• Maintain community involvement to identify community needs and enhance working relationships with community resources, groups and individuals.
• Offer program services to families based on screening results.
• Identify and maintain contact with community services to inform clients of benefits, programs and services.
• Maintain knowledge of current community resources.
• Develop and/or maintain an organized data system of client interaction and follow-up.
• Provide short-term case work and case management services.
• Provide monthly and quarterly reports.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED TO PERFORM IN THIS POSITION:
Responds to requests for information and assistance from the community and individuals.
Provides assistance, education, and referral to resources.
Coordinates services with, initiates referrals to, and accepts referrals from other providers and other Health District programs.
Consults with supervisor on “uncommon requests” and follows his/her recommendation.
Participates in community health promotion efforts including preconception health.
Provides appropriate documentation.
Documents services provided promptly and on correct record forms.
Records in a clear, concise, objective manner.
Reviews documents to be sure all recording is complete.
Communicates clearly; if not bilingual obtains assistance of bilingual person for interpretation.
Spanish speaking is highly desirable.
QUALIFICATIONS:
Ability to communicate with individuals and the public. Must have basic knowledge of health education and promotion. Graduation from an accredited four-year college or university with major coursework in a field relevant to assignment is preferred. Experience and education may be substituted for one another.

May be exposed to communicable diseases.
Must be fully immunized within 90 days of employment and show proof thereof.
Must be able to spend six to eight hours walking and standing and/or sitting.
Must be able to lift up to 25 pounds and stretch to reach files.
