JOB SUMMARY:
Responsible for exemplifying NET Health’s (North East Texas Public Health District) Mission, Vision and Values, providing outstanding customer service. The Community Resource Specialist (CRS) will function as a member of a multidisciplinary team that provides education, information, outreach and referrals for preconception health, pregnant and post-partum women who are within WIC Clinic Community or WIC eligible.
MAJOR TASKS AND RESPONSIBILITIES:
• Arrange client-confirmed services.
• Monitor and document client needs, progress, and success.
• Provide education on health-enhancing behaviors and chronic disease management and prevention.
• Serve as family advocate, identifying and reducing barriers to health care.
• Maintain community involvement to identify community needs and enhance working relationships with community resources, groups, and individuals.
• Offer appropriate services to families based on screening results.
• Identify and maintain contact with community services to inform clients of benefits, programs, and services.
• Develop and/or maintain an organized data system of client interaction and follow-up.
• Provide short-term case work and case management services.
• Provide monthly and quarterly reports.
• Participate in staff training topics such as risk assessment tools, care coordination models, data collection tools, and maternal morbidity in Texas.
• Attend other necessary trainings and meeting related to job requirements and expectations.
• Work with DSHS, Healthy Texas Mothers and Babies and High-Risk Care Coordination team members on special projects such as reviewing Maternal Health CHW curriculum, testing modules of CHW curriculum, providing feedback and assisting with special events for the mother, infant, child and whole family.

KNOWLEDGE, SKILLS, ABILITIES AND COMPETENCIES:
• Knowledge of community resources and experience working with a variety of community-based organizations preferred.
• Experience in conducting community outreach.

• Flexibility in managing a variety of types of assignments.
• Meet deadlines for assignments.
• Knowledge of preconception health and ability to educate and engage in health discussions.

• Spanish-speaking preferred.
REQUIRED EDUCATION / EXPERIENCE:
• High School diploma or GED required. Two years of health/clinical experience or equivalent health-related education required.
• Current Community Health Worker certification by the Texas Department of State Health Services. Must maintain current CHW certification.
• CPR certification preferred or obtained within 3 months of hire. Must maintain current CPR certification.

May be exposed to communicable diseases.
Must be fully immunized within 90 days of employment and show proof thereof.
Must be able to spend six to eight hours walking and standing and/or sitting.
Must be able to lift up to 25 pounds and stretch to reach files.
