Your name and contact information will remain confidential to the extent law permits.  We will not share your identity with the facility.  Providing your contact information helps us follow up with you in case we need more information or to let you know what actions were taken due to our investigation.  Court cases are rare, but if a complaint is taken to court, portions of the complaint that cannot be redacted could be presented to the court at that time.

General Complaint Form:

This form is designed for the public to report environmental health concerns and public nuisances that may impact community well-being. Examples of issues that can be reported include:

  • Pest infestations or vermin
  • Illegal dumping or improper waste disposal
  • Overflowing trash or debris accumulation
  • Sewage leaks or improper sewage disposal
  • Structural concerns at permitted facilities

Please note, this is not an all-inclusive list. There may be other concerns not listed here that also fall under public health or environmental safety. Use this form to help NET Health address and resolve these issues, ensuring compliance with health and safety regulations. For other specific concerns, please refer to our specialized complaint forms.

Chapter 341 of the Health and Safety Code provides for the regulation of public health hazards.

Report Food Borne Illness or Injury:

Step 1 – Complete the Microsoft Form to file a complaint with NET Health of a suspected Foodborne Illness (Link to Microsoft FBI Form) 

Step 2 – NET Health will review the data and may contact you back to collect more details about the scenario.  If you became ill, you will likely be asked to complete a 3 day food history and in some cases up to 2 weeks of foods and beverages consumed.  Some tips and tricks to recall what you have consumed are:  review receipts, credit card transactions, phone calls and calendar events.  Food Borne Illness Complaint FBI Complaint Form.pdf 

Step 3 – Complaint data will be compared to other complaints to associate common source and to schedule investigations 

Step 4 – For multiple complaints from a common source, NET Health will initiate a foodborne illness outbreak investigation.
