D.R.E.A.M. is a care coordination program for high-risk expecting and postpartum mothers that reduces maternal morbidity and mortality by identifying and reducing barriers to quality health care for expecting and postpartum women.
Our goal is to close those gaps by equipping and empowering mothers and their families with the knowledge, tools, and community resources to make informed decisions throughout their pregnancy and postpartum journey.
